It sounds great at first. Companies like Headway, Alma, Betterhelp and others promise to handle billing, credentialing, and contracts, things that feel overwhelming without a clinic behind you. And yes, those things do take work and expertise.
But that doesn’t justify giving up a large portion of your income indefinitely.
At some point, most therapists start asking, “how much pay is being kept from me?"
If you want a simple way to check, look up CPT code 90837 in your locality using the Medicare fee schedule. In Minnesota, the non-facility rate is the baseline most outpatient therapy is built on. Master’s-level clinicians are reimbursed at about 75% of that rate, and commercial insurance plans typically pay 25–35% more, while Medicaid often pays 30–50% less.
Once you see those numbers, it becomes much clearer what a session is actually worth, and how much may be getting taken out before it reaches you.
